Many risks become threats, which then become problems, for one reason: a lack of imagination! PMs and engineers simply cannot imagine outlier events or event-combinations, because they have never seen those events before. And they are untrained in the "what-if" techniques that I have used throughout my professional life. I am known for asking tough questions and postulating possible future situations that make my clients' PMs, BusDev and other staffers squirm uncomfortably. But their projects and programs do NOT get cancelled and we are still friends!
